Dog Adoption Questionnaire

Thank you for applying to adopt a rescue dog and saving a life!

PLEASE NOTE that completion of this questionnaire does not guarantee the adoption of a rescue dog from Sweet Pea Animal Rescue (SPAR). The questionnaire is a tool designed to help SPAR source the best possible forever homes for the dogs that we rescue.

Upon completion of the questionnaire and prior to any decision being made, a ‘secure yard check’ must be completed by SPAR. If the application is successful, the adoption fee must be paid in full prior to adoption. All SPAR rescue animals are subject to a two week trial. If you decide during the two week trial that you do not want to keep the dog, we will happily refund your adoption fee, minus a $50 administration fee, when the dog is returned.

In some cases, more than one application will be received for the adoption of the same rescue dog. The decision is then with SPAR as to which application is the most suitable for the dog in question. If the dog you apply for is adopted by someone else, we are happy to try and help you find another suitable dog to adopt. However, no guarantee can be given that a suitable dog will be found.
